Enter your name and state in the form, then click the “Search” button.

Find the correct match for your personal information in the search results, then click the “Proceed to Opt Out” button.

Enter your email address, check the CAPTCHA box, then click the “Send Verification Email” button.

Open the verification email you receive and click “Verify Opt-Out” to confirm your request.

You will receive a message that your request has been submitted. Your information will be removed from the site within about 24 hours.
