Scroll to the bottom of the page until you reach the section labeled, “Why Am I Listed?/Removal Instructions”.

In the form, enter in your telephone number that you’d like to have removed and the zip code associated with the phone number.

To complete the request, click on “Request Removal”, and they will aim to have your request processed within 48 hours during normal business hours.
